Fund Raising Campaign Policy

Objective:

To help co-members lessen his/her financial burdens in times of calamities or health problem.

Coverage:

  1. Health-related requirements in order to defray the cost of medicines and medical processes for members and his immediate dependents (Spouse, Parents, Children, Brothers & Sisters);
  2. Coop member’s house affected by natural calamities such as flashfloods, super typhoons, fire, etc.

Requirements:

Member who wishes to avail the Fund raising must comply with the following requirements:

  1. He/She must be at least 3 months coop member and in good standing.
  2. He/She has exhausted all others means such as availing of company benefits and coop loans before seeking fund raising assistance.
  3. Written request address to the Board of Directors signed by the requestor and his/her immediate supervisor.  The request must indicate the case, the description or the extent of the damage brought about by the case;
  4. Copy of webapps
  5. Submit copy of applicable supporting documents as proof of his/her case.

      • Medical abstract
      • Billing statement
      • Medical certificate
      • Quotation from hospital
      • Other medical related documents
    • Property Related Incident
      • Picture of the destroyed property
      • Certification from barangay
      • Proof of ownership
      • Other related documents

Procedure:

  1. The Administrator and BOD will review, evaluate and decide whether to provide assistance through fund raising or forego the request;
  2. BOD approval is required prior to announcement of fund raising campaign.
  3. Coop member may donate cash or thru salary deduction voluntarily.  Member is required to fill up and submit the Fund Raising Pledging form.
  4. Fund raising campaigns for salary deductions is limited up to 4 paydays only.
  5. Once collected, the amount will be turned-over to the member requestor.

Coop Share:

STMC will contribute P5,000 to P20,000 for every approved fund raising to be given to requesting member.  The said amount shall be taken out from community development fund.

Condition Coop Share
If member-patient has (1) one of the 6 critical illnesses or  an ICU cases Php 20,000
   
   
If member-patient disease is not among 6 critical illness or an ICU cases. Php 5,000
If patient is dependent Php 5,000

The 6 critical illnesses are the following:

  1. Chronic Renal Failure – the end stage renal failure presenting a chronic irreversible failure of both kidneys to function, as a result of which regular renal dialysis is instituted.
  2. Coronary Artery Bypass Surgeon – the actual undergoing of open chest surgery for the correction of two or more coronary arteries, which are narrowed or blocked by coronary artery bypass graft (CABG), causing inadequate myocardial blood supply.  The surgery must have been proven by means of the coronary angiography.  Laser therapy angioplasty and or any other intra arterial procedures are excluded from this definition.
  3.  Myocardial Infarction Surgery – the death of a portion of the heart muscle as a result of inadequate blood supply.  Diagnosis must be on:
    • Electrocardiographic changes; and
    • Higher level of cardiac enzymes above standard laboratory level of normal
  4. Major Organ Transplant Surgery – the human to human organ transplant from donor to the member as donee, of one or more of the following organs – kidney, heart, lung, liver, pancreas, or the transplantation of bone marrow.  The transplantation of another organ, only part of an organ or any other tissue transplant are excluded from this definition.
  5. Malignant cancer – the presence of one or more malignant tumors, including malignant lymphoma, Hodgkin’s disease, leukemia and malignant bone marrow disorders, and is characterized by the uncontrolled growth and spread of malignant cells and the invasion and destruction of normal tissue, but does not include the following:
    • Tumors which are histologically described as pre-malignant or showing the malignant changes of carcinoma in situ and not requiring radical surgeon;
    • Skin cancers are melanomas except where a malignant melanoma is equal to a greater than clark level 3 or 1.5m depth or invasion: or
    • prostatic cancers which are histologically described as TNM Classification T1 or are of another equivalent or lesser classification.
  6. Stroke – any cerebrovascular accident or incident producing neurological sequelae lasting more than twenty-four (24) hours.  This includes infraction of brain tissue, intracranial or subarachnoid hemorrhage, embolisation from an extracranial source.  Transient ischemic attacks, cerebral symptoms due to migraine and vascular disease affecting the eye or optic nerve are excluded.

Penalty Clause:

  1. If the beneficiary used the money not for its intended purpose, the BOD may declare a member as a member not in good standing and therefore no longer allowed to enjoy member’s benefit.
  2. A member maybe terminated by a vote of the majority of all the members of the board of directors for falsification of documents, any act omission injurious or prejudicial to the interest or the welfare of the cooperative.
